Submarine Crash

I have been waiting to see what the situation was and now the <a href="http://abcnews.go.com/US/wireStory?id=738316&CMP=OTC-RSSFeeds0312" target="_new">report is out</a>. The Submarine crash that took a life, injured others, and took a Submarine out of commission was an <b>avoidable</b> accident that never should have happened.
